本课程系统介绍职业环境因素对劳动者健康的影响及预防控制措施。内容涵盖职业性有害因素识别与评价(粉尘、化学毒物、物理因素、生物因素、工效学因素)、职业流行病学调查、职业卫生监测、职业病诊断与管理、职业卫生服务体系、职业安全与防护以及职业卫生法律法规。通过案例分析和现场调查实践,培养学生职业卫生风险评估与控制的能力。

This course systematically introduces the impact of occupational environmental factors on worker health and prevention control measures. Topics include occupational hazard identification and evaluation (dust, chemical toxins, physical factors, biological factors, ergonomic factors), occupational epidemiology surveys, occupational health monitoring, occupational disease diagnosis and management, occupational health service systems, occupational safety and protection, and occupational health laws and regulations.

• 接触评估方案:确定接触评估方法(环境监测、生物监测、问卷评估、接触指数法、职业接触分类),制定采样策略(个体采样、区域采样),确定检测指标和采样频率

步骤 2
工作场所有害因素检测与接触评估
本步骤的核心任务是系统检测工作场所职业有害因素的浓度/强度,评估劳动者的接触水平。需要按照国家职业卫生标准和检测规范,使用专业仪器设备进行现场采样和实验室检测,确保检测数据的准确性和可靠性。接触评估是职业健康风险评估的基础。

• 现场检测与采样:使用粉尘采样器(总尘/呼尘)、大气采样器、声级计、WBGT指数仪、振动测量仪等仪器进行现场检测,按照GBZ系列标准确定采样点、采样时间和采样流量
• 实验室分析:采集的样品送实验室分析,使用气相色谱、液相色谱、原子吸收光谱、ICP-MS等方法测定有害物质浓度,进行质量控制(空白对照、平行样、加标回收)
• 接触评估:计算时间加权平均容许浓度(TWA)、短时间接触容许浓度(STEL)、最高容许浓度(MAC),与职业接触限值(OELs)比较,评估接触水平超标情况,绘制接触分布图

职业健康检查与健康效应评估
本步骤的核心任务是对接触职业有害因素的劳动者进行职业健康检查,评估有害因素对健康的影响。需要按照职业健康监护技术规范,开展上岗前、在岗期间、离岗时的职业健康检查,早期发现职业健康损害。职业健康检查是预防职业病的重要手段。

• 职业健康检查:按照GBZ 188职业健康监护技术规范,针对不同有害因素确定检查项目——粉尘接触者做胸片和肺功能、噪声接触者做纯音测听、毒物接触者做血尿常规和肝肾功能、物理因素做相应专科检查
• 健康效应评估:分析接触人群与对照人群的健康指标差异,计算职业病、职业相关疾病、亚临床损害的发生率,分析剂量-反应关系(接触水平-健康效应),确定健康损害的职业关联性
• 职业病诊断与报告:对疑似职业病患者进行职业病诊断(按照职业病诊断标准和诊断程序),建立职业健康监护档案,按照规定报告职业病和疑似职业病

步骤 4
职业健康风险评估与控制措施制定
本步骤的核心任务是评估职业有害因素的健康风险,制定工程控制、管理控制和个体防护等综合防控措施。需要根据有害因素的危害程度和接触水平,采用定性或定量风险评估方法确定风险等级,按照优先顺序制定控制措施。风险评估是职业健康管理的核心工具。

• 风险评估:采用风险评估矩阵法、半定量风险评估模型或定量风险评估(QRA)方法,综合考虑危害严重性、接触水平、接触时间、易感性等因素,确定风险等级(低风险/中风险/高风险/极高风险)
• 控制措施制定:按照控制措施优先顺序制定——工程控制(通风排毒、密闭隔离、隔声减振、自动化)、管理控制(操作规程、教育培训、接触时间限制、健康监护)、个体防护(防尘口罩、防毒面具、耳塞、防护服装)
• 控制效果评价:预测和评估控制措施的预期效果,进行成本-效果分析,制定分阶段实施计划,提出应急救援措施(急救设施、应急装备、应急预案)

职业卫生调查报告撰写与职业健康管理体系建设
本步骤的核心任务是撰写职业卫生调查报告,提出职业健康管理体系建设建议。需要系统总结调查发现,评估用人单位的职业卫生状况,提出改进建议和管理体系建设方案。职业健康管理体系的建立和运行是持续改善工作条件的制度保障。

• 撰写调查报告:按照职业卫生调查报告规范撰写,包括企业概况、生产工艺、有害因素识别、检测结果、健康检查结果、风险评估、问题分析、改进建议,附检测报告和健康检查报告
• 职业健康管理体系建设:依据GBZ/T 297或ISO 45001标准,设计职业健康安全管理体系,包括方针目标、组织机构、管理制度、操作规程、应急预案、培训计划、档案管理等要素
• 持续改进机制:建立职业健康定期检查和评价制度,制定持续改进计划,提出职业病防治规划(三年/五年),建立监测预警和健康促进机制

Step 1
Occupational Hazard Identification and Occupational Health Survey Design
The core task of this step is to identify occupational hazards in the workplace and design a scientific occupational health survey plan. Comprehensively identify various occupational hazards from production processes, labor processes, and production environment.

• Occupational hazard identification: analyze chemical factors (dust, toxic substances, organic solvents, heavy metals), physical factors (noise, high temperature, vibration, ionizing radiation, non-ionizing radiation), biological factors (bacteria, viruses, fungi), ergonomic and psychosocial factors in production processes
• Occupational health survey design: determine survey type (cross-sectional, case-control, cohort, occupational health examination); select industries and job types; determine exposed and control populations; develop survey and testing plans
• Exposure assessment plan: determine exposure assessment methods (environmental monitoring, biomonitoring, questionnaire assessment, exposure index, occupational exposure classification); develop sampling strategy (personal sampling, area sampling); determine test indicators and sampling frequency

Step 2
Workplace Hazard Detection and Exposure Assessment
The core task of this step is to systematically detect concentrations/intensities of occupational hazards in the workplace and assess workers' exposure levels. Follow national occupational health standards and testing specifications.

• On-site detection and sampling: use dust samplers (total/respirable dust), air samplers, sound level meters, WBGT index meters, vibration meters for on-site testing; determine sampling points, time, and flow rates per GBZ standards
• Laboratory analysis: send collected samples to lab for analysis; use GC, LC, AAS, ICP-MS to determine hazardous substance concentrations; perform quality control (blank controls, parallel samples, spike recovery)
• Exposure assessment: calculate TWA, STEL, MAC; compare with occupational exposure limits (OELs); assess exceedance of exposure levels; draw exposure distribution maps

Step 3
Occupational Health Examination and Health Effect Assessment
The core task of this step is to conduct occupational health examinations for workers exposed to occupational hazards and assess health impacts. Perform pre-employment, during-employment, and post-employment exams per technical specifications.

• Occupational health examination: follow GBZ 188 technical specifications for occupational health surveillance; determine examination items for different hazards—chest X-ray and lung function for dust exposure, pure-tone audiometry for noise, blood/urine routine and liver/kidney function for toxic exposure, specialty exams for physical factors
• Health effect assessment: analyze health indicator differences between exposed and control populations; calculate incidence of occupational diseases, work-related diseases, subclinical damage; analyze dose-response relationship; determine occupational relevance of health damage
• Occupational disease diagnosis and reporting: perform occupational disease diagnosis for suspected cases (following diagnostic standards and procedures); establish occupational health surveillance files; report occupational diseases and suspected cases per regulations

Step 4
Occupational Health Risk Assessment and Control Measure Development
The core task of this step is to assess health risks of occupational hazards and develop comprehensive control measures including engineering controls, administrative controls, and personal protective equipment.

• Risk assessment: use risk matrix, semi-quantitative risk assessment model, or quantitative risk assessment (QRA); comprehensively consider hazard severity, exposure level, exposure duration, susceptibility; determine risk level (low/medium/high/very high)
• Control measure development: develop measures following hierarchy of controls—engineering controls (ventilation, enclosure, sound insulation, automation), administrative controls (SOPs, education/training, exposure time limits, health surveillance), personal protective equipment (dust masks, respirators, earplugs, protective clothing)
• Control effectiveness evaluation: predict and evaluate expected effectiveness of control measures; perform cost-effectiveness analysis; develop phased implementation plan; propose emergency rescue measures (first aid facilities, emergency equipment, emergency plans)

Step 5
Occupational Health Investigation Report Writing and OH&S Management System Establishment
The core task of this step is to write an occupational health investigation report and propose occupational health management system construction recommendations. Systematically summarize findings and assess enterprise occupational health status.

• Write investigation report: follow occupational health report standards, including enterprise overview, production process, hazard identification, testing results, health examination results, risk assessment, problem analysis, improvement recommendations; attach testing and health exam reports
• OH&S management system establishment: design occupational health and safety management system per GBZ/T 297 or ISO 45001; include policy objectives, organizational structure, management systems, operating procedures, emergency plans, training plans, file management
• Continuous improvement mechanism: establish regular occupational health inspection and evaluation system; develop continuous improvement plan; propose occupational disease prevention plan (3-year/5-year); establish monitoring, early warning, and health promotion mechanisms
