本课程聚焦特殊需要学生从学校到成人生活的转衔教育理论与实践。课程内容包括转衔的概念与意义、转衔服务的法律框架与政策要求、个别化转衔计划(ITP)的制定、转衔评估的方法与工具、职业教育与就业准备、独立生活技能培养、社区参与与社交技能发展、高等教育机会的获取、以及家庭与社区资源的链接。学生将学习如何与学生、家庭、雇主及社区机构协作,为特殊需要学生的中学后生活做好充分准备,促进其成功融入社会。

This course focuses on the theory and practice of transition education for students with special needs moving from school to adult life. Topics include transition concepts and significance, legal frameworks and policy requirements, Individualized Transition Plans (ITP), transition assessment methods and tools, vocational education and employment preparation, independent living skills, community participation and social skills development, postsecondary education access, and family and community resource linkage. Students learn to collaborate with students, families, employers, and community agencies to prepare students for post-school success and social inclusion.

作业标题:个别化转衔计划(ITP)制定

学生需为一名即将从高中过渡到成人生活的特殊需要学生(模拟案例),制定一份完整的个别化转衔计划。计划内容包括:学生现状评估(学业成就、职业兴趣与技能、独立生活能力、社交能力等)、转衔目标设定(就业、教育、独立生活、社区参与等领域)、具体行动策略与支持服务、合作机构与人员分工、转衔活动时间表、以及成效评估方法。要求以学生为中心,结合其兴趣和能力,设计可操作且有支持的转衔路径,体现对学生自主决策和自我倡导能力的培养。

Students will develop a comprehensive Individualized Transition Plan (ITP) for a student with special needs transitioning from high school to adult life (simulated case). The plan should include current assessment (academic achievement, vocational interests and skills, independent living abilities, social skills), transition goals across employment, education, independent living, and community participation domains, specific action strategies and support services, collaborative partners, timeline of transition activities, and evaluation methods. Student-centered planning and development of self-determination and self-advocacy skills should be emphasized.
